2. Pre-Session Form
Your dietitian receives this form before your session and is better prepared when the session begins which saves you time and money. It also helps protect our liability.
Take your time when completing this form. Most of our clients tell us that this process helps them prepare for their session and gives them valuable insights into the issues they want to address.

Note: If you are purchasing a supplement review, diet analysis, or meal plan, you will not use this pre-session form! You must first purchase this service through our scheduling software by registering your customer account and purchasing the service through the shopping cart. After you have done this, please click here to fill out a supplement review form or click here to fill out our dietary recall sheet. Your supplement review form or dietary recall form will not be valid until payment has been received!
3. Schedule Your Appointment

here to schedule your appointment after you have paid for your session and filled out your pre-session form. You may also call us at 1-800-980-9780 and have a Nutradvice customer support specialist schedule your appointment for you. For scheduling of phone consultations, review your dietitian's calendar and select the time that you want from their openings. Please make sure to take the dietitian's time zone in to account if you do not share the dietitian's time zone. The appointment will be scheduled according to their time zone of the dietitian. Use

For email consultations, there is no need to schedule an appointment with your dietitian. You will purchase this service through our scheduling software by registering your customer account and purchasing the service through the shopping cart. Your dietitian will receive your email questions from the pre-session form you filled out.
